Heparin (LMWH - Enoxaparin)

Mechanism of action

Anticoagulant (low molecular weight heparin).

Potentiates action of antithrombin III on factor Xa, inhibiting clot formation.

Avoid if creatinine clearance <15 ml/minute.

Consult literature if creatinine clearance 15-30 ml/minute.
